Transition

Transition is the process of preparing students for life after high school, including successful participation in postsecondary education or training, employment and community living. These three "post-school outcomes " should provide the driving focus for IEP teams as they develop plans for secondary-aged students.  Transition planning begins when a student is 14 and must be documented on the IEP annually when a student reaches this age.
