Paperwork to register for homeschool or for ongoing student(s) must be received on or before the August 1st deadline every year

    1. Copy of H.S. or College diploma of homeschool supervisor giving instruction (needed for each student when first enrolling)
    2. Objectives – brief outline of subjects being taught. Needed for each student each year
    3. Signed, notarized Affidavit for each student (can combine more than 1 student on the same form)

    4.  Medical/Dental
      1. Medical Info: immunization records or letter/form exempting “due to religious beliefs”

      2. Dental Records:  Current exam due upon enrollment, 3rd and 7th grades
        1. Dental Form
      3. Physical Form:  Current exam due upon enrollment, 6th and 11th grades
        1. Physical Form

    An end of year evaluation must be done for each homeschool student by a certified evaluator. The evaluator will supply a report and review any necessary testing.  Parents may drop off or mail the evaluator’s report to Mrs. Monica Fisher in the High School Guidance office on or before the June 30th deadline for each year.
